About this equipment

BiPAP delivers two pressures — a higher one as the patient breathes in and a lower one as they breathe out. That difference is what makes it tolerable for hours at a time and what makes it work where plain CPAP does not: in COPD, in obesity hypoventilation, in neuromuscular weakness, and in anyone who has to be helped to breathe out as well as in. The machine is the straightforward part. The mask is not: mask leak is the single commonest reason home therapy gets abandoned, so we fit it in person and try the styles rather than assuming a medium.

The things that are easy to find out too late.

  • The mask matters more than the machine

    Nearly every abandoned BiPAP is a mask problem, not a machine problem. We re-fit free within the first two weeks, which is when the trouble shows up. Tell us rather than tightening the straps — over-tightening causes the nasal bridge sores it is meant to prevent.

  • Pressures are prescribed, not chosen

    IPAP and EPAP come from a sleep study or a blood gas assessment. We will not supply a machine on guessed settings, because the wrong pressure in a retaining patient is dangerous rather than merely ineffective.

  • Distilled water only

    Tap water scales the humidifier chamber and the mineral residue ends up in the airway. This is the most common maintenance failure we see.

  • Consumables are ongoing

    Mask cushions last around three months and filters around a month. Budget for them — a perished cushion leaks, and a leaking mask is an untreated patient.

How it is installed and used

The mask is fitted to the patient's face in person — nasal, nasal-pillow and full-face styles are tried rather than a medium being assumed, because mask leak is the single most common reason home therapy is abandoned. Pressures, mode and ramp are set to the clinician's prescription, the humidifier is filled with distilled water, and the settings are confirmed before the technician leaves.

Before delivery

A sleep study or blood gas assessment and clinician-set pressures are required before delivery. Machines are not supplied on guessed settings. Confirm mains supply and, for dependent patients, that an inverter can carry the unit overnight.

Ongoing care and servicing

Mask cushions are replaced roughly every three months and filters monthly. Compliance data is available on the device for the treating clinician. We re-fit the mask free within the first two weeks, which is when most fit problems surface.

When not to use it

Untreated pneumothorax
Positive airway pressure can enlarge a pneumothorax. Non-invasive ventilation is contraindicated until it is treated.
Impaired consciousness or inability to protect the airway
A patient who cannot remove the mask or clear secretions is at risk of aspiration; these patients need supervised or invasive ventilation instead.
Recent facial, upper airway or oesophageal surgery
Mask pressure and insufflation can disrupt healing at the surgical site.
Active vomiting or significant haemoptysis
Positive pressure with a sealed mask substantially raises aspiration risk.

Possible side effects

Mask leak and pressure sores on the nasal bridge
The commonest problem in home therapy. Usually a sizing or style mismatch rather than user error — which is why we re-fit rather than tell people to tighten the straps.
Aerophagia and abdominal distension
Swallowed air causes bloating and discomfort, often improved by adjusting the expiratory pressure.
Dryness, congestion and eye irritation
From leak past the mask seal and inadequate humidification. Use distilled water and check the seal.

Serious risks

Life-threatening, permanently damaging, or requiring hospitalisation.

Aspiration of gastric contents
In a drowsy or vomiting patient a sealed mask can drive gastric contents into the lungs.
Tension pneumothorax
Positive pressure applied over an undiagnosed pneumothorax can become rapidly life-threatening.

What is the difference between BiPAP and CPAP?
CPAP holds one constant pressure. BiPAP uses a higher pressure to breathe in and a lower one to breathe out, which makes it tolerable at higher pressures and lets it help patients who struggle to exhale — COPD, obesity hypoventilation, neuromuscular weakness. CPAP is usually first-line for straightforward obstructive sleep apnoea.
Do I need a sleep study before renting a BiPAP machine?
You need clinician-set pressures, which normally come from a sleep study or a blood gas assessment. Encone Care runs home sleep studies if you do not have one yet.
Can I rent a BiPAP machine to try it first?
Yes, and it is a sensible way to start — tolerance varies a lot between patients, and rental lets you find out before buying. Call us for a quote covering the trial period.
How often do the mask and filters need replacing?
Mask cushions roughly every three months, filters roughly monthly. Both are ordinary consumables and both are cheap relative to the cost of therapy failing because of a leak.
