About this equipment

CPAP holds one steady pressure through the whole breath, splinting the upper airway open so it cannot collapse during sleep. For straightforward obstructive sleep apnoea it is the first-line treatment and it works well — patients who stick with it usually report the difference within a fortnight. Sticking with it is the whole problem, and it comes down to the mask and the humidifier rather than the machine. We fit the mask in person and set the humidity, because a dry throat at 3 AM is how CPAP ends up in a cupboard.

  • The first two weeks are the hard part

    Most people who give up on CPAP do so early, over mask fit or dryness. Both are fixable. Call us during that fortnight instead of stopping — we re-fit the mask free.

  • Fixed pressure, not automatic

    This is a fixed-pressure machine set to your prescription. If your pressure requirement varies through the night, ask the treating clinician about an auto-titrating unit instead.

  • Distilled water only

    Tap water scales the humidifier chamber and leaves mineral residue in the air path. It is the most common maintenance failure we see.

  • It treats, it does not cure

    Apnoeas return the night you stop using it. CPAP is ongoing therapy, which is why long-term users usually buy rather than rent.

How it is installed and used

The mask is fitted in person — nasal, nasal-pillow and full-face styles are tried rather than a medium being assumed. The fixed pressure is set to the clinician's prescription, ramp and humidity are configured, the chamber is filled with distilled water, and the patient wears the mask under pressure before the technician leaves.

Before delivery

A sleep study or blood gas assessment and clinician-set pressures are required before delivery. Machines are not supplied on guessed settings. Confirm mains supply and, for dependent patients, that an inverter can carry the unit overnight.

Ongoing care and servicing

Mask cushions are replaced roughly every three months and filters monthly. Compliance data is available on the device for the treating clinician. We re-fit the mask free within the first two weeks, which is when most fit problems surface.

When not to use it

Untreated pneumothorax
Positive airway pressure can enlarge a pneumothorax. Non-invasive ventilation is contraindicated until it is treated.
Impaired consciousness or inability to protect the airway
A patient who cannot remove the mask or clear secretions is at risk of aspiration; these patients need supervised or invasive ventilation instead.
Recent facial, upper airway or oesophageal surgery
Mask pressure and insufflation can disrupt healing at the surgical site.
Active vomiting or significant haemoptysis
Positive pressure with a sealed mask substantially raises aspiration risk.

Possible side effects

Mask leak and pressure sores on the nasal bridge
The commonest problem in home therapy. Usually a sizing or style mismatch rather than user error — which is why we re-fit rather than tell people to tighten the straps.
Aerophagia and abdominal distension
Swallowed air causes bloating and discomfort, often improved by adjusting the expiratory pressure.
Dryness, congestion and eye irritation
From leak past the mask seal and inadequate humidification. Use distilled water and check the seal.

Serious risks

Life-threatening, permanently damaging, or requiring hospitalisation.

Aspiration of gastric contents
In a drowsy or vomiting patient a sealed mask can drive gastric contents into the lungs.
Tension pneumothorax
Positive pressure applied over an undiagnosed pneumothorax can become rapidly life-threatening.

Frequently asked questions

How long before CPAP starts working?
Most patients notice better daytime alertness within one to two weeks of consistent nightly use. The therapy only works on nights you use it — benefits do not accumulate through gaps.
Should I rent or buy a CPAP machine?
Rent if you are trialling therapy or unsure about tolerance. Buy if you have an established diagnosis and a pressure that works, since sleep apnoea does not resolve on its own. Ask us for a quote on both and we will lay out the comparison.
Can I use tap water in the humidifier?
